303 SCREENING INSTRUMENTS TO PREDICT ADVERSE OUTCOMES FOR UNDIFFERENTIATED OLDER ADULTS ATTENDING THE EMERGENCY DEPARTMENT: RESULTS OF SOAED PROSPECTIVE COHORT STUDY

Abstract Background Frailty screening facilitates the stratification of older adults at most risk adverse events for urgent assessment and subsequent intervention in acute or community setting. We assessed validity ISAR (Identification Seniors Risk), Rockwood Clinical Scale (CFS), PRISMA-7 InterRAI-ED predicting outcomes 30 days six months among presenting to ED. Methods A prospective cohort study consecutive (≥65 years) who presented ED a University Hospital was conducted. The ISAR, CFS, were performed by an experienced research nurse. Blinded follow-up telephone interviews completed assess incidence mortality, re-attendance, hospital readmission, functional decline nursing home admission. sensitivity specificity tools calculated using 2×2 tables. Results 419 patients recruited with 49% female mean age 76.9 years (SD 7.15). prevalence frailty varied across (ISAR, 47% vs InterRAI-ED, 63%). At 30-days, mortality rate 5.4%, re-attendance 16.9%, readmission 13.6%, 47.1% admission 7.3%. Older screened positive demonstrated increased all 6 months, regardless tool administered.
